Neuroendocrine Tumour Treatment and Wellness Care Plan

We are very pleased to announce the arrival of a new (and much anticipated) resource, our Neuroendocrine Tumour Treatment and Wellness Care Plan.

This project is the cumulation of a two-year project, in collaboration with the Australian Cancer Survivorship Centre – A Richard Pratt Legacy (ACSC). The Treatment and Wellness Plan is designed to be used by patients as a record of their NET diagnosis, treatment and team, and a reminder of special instructions to follow.

The care plan begins with general information (background information) and is then divided into 3 three sections as follows:

  • Treatment summary (a record of treatment to date)
  • Follow-up plan (information about follow-up appointments and what to expect)
  • Living with NETs (wellness plan)

The resource can be downloaded, free of charge from our website here NET Treatment and Wellness Care Plan and either filled in using your computer, or printed and filled in by hand. It can be completed by patients, with assistance of their health professionals (as available), and is designed to be kept by patients as an ongoing resource.

As this is a new resource, we are very keen to get feedback from both patients and health professionals. Please help us by filling out the survey (should only take five minutes of your time):
