Thank you for choosing to work with our dietitian. This initial consultation is a key step in creating a nutrition plan that’s tailored to your needs and goals.
Please take a few moments to fill out the intake form below, as this will help your dietitian get a clear understanding of your health background and current needs. If you’re unable to complete it beforehand, no problem, your dietitian will go through it with you during the appointment.
Patient Information for the Dietitian Clinic
To make the most of your first appointment, please provide as much information as possible in the online intake form. This information allows our dietitian to prepare for your consultation and offer the best possible support.
If you’re comfortable sharing any doctors’ letters, operation reports, or discharge summaries that may be relevant, please email these directly to us, or upload into the form below. These documents can provide further insights that support your dietitian’s recommendations.

- Your Diagnosis
- Current Treatment
- Previous Surgeries and Treatments
- Full Medical History, including any conditions such as diabetes, high cholesterol, heart disease, eating disorders, bowel surgery, etc.
- List of Current Medications
- Recent Blood Test Results (if accessible)
What to Expect at Your First Dietitian Appointment
Your first appointment with the dietitian will focus on understanding your health needs, current lifestyle, and nutritional goals. You’ll discuss your medical history, diet, and any specific challenges you’re facing. Our dietitian will work closely with you to create a realistic and achievable plan that supports both your health and lifestyle.

Multilingual Line Support Details
NeuroEndocrine Cancer Australia are pleased to announce that we can now offer our NET Nurse service to those who speak a language other than English, via the National Translating and Interpreting Service, for the cost of a local call (except for mobiles).
To access this Translating and Interpreting Service (TIS National), simply follow these steps:
- Call 13 14 50, Monday to Friday 9.30 am – 4.30 pm.
- Say the language you need.
- Wait on the line for an interpreter (may take up to 3 minutes).
- Ask the interpreter to contact NeuroEndocrine Cancer Australia on 1300 287 363
- Talk with our NET Nurse with the help of an interpreter.
